Assistive technology and home modifications provide equipment, products and practical changes to your home that help you live safely and independently.
Depending on your needs, this may include mobility equipment such as walking frames or wheelchairs, as well as home modifications such as handrails in bathrooms and other areas.
The goal is to make everyday activities easier and safer, helping you maintain your independence, accessibility and quality of life for longer.
This support is available through the Assistive Technology and Home Modifications (AT-HM) Scheme, which is part of the Support at Home program.

What Help Is Available?
The AT-HM Scheme provides support across two main areas:
1. Assistive Technology
Assistive technology includes products and equipment designed to make everyday activities safer, easier and more independent.
This may include:
- Wheelchairs, walking frames and walking sticks
- Toileting aids such as commodes
- Shower chairs and bath boards
- Communication aids
- Adaptive kitchen and eating equipment
2. Home Modifications
Home modifications are practical changes that can make your home safer and easier to access, helping you remain independent at home for longer.
This may include:
- Lever taps and door handles
- Grab rails in bathrooms and showers
- Indoor and outdoor handrails
- Ramps and stair lifts
- Non-slip flooring and safety materials
The AT-HM List outlines the approved products, equipment and home modifications available through the scheme.
Funding information
Funding through the AT-HM Scheme is based on your individual needs and the level of support required.
| Funding Tier | Funding Amount | Access Period |
|---|---|---|
| Low | Up to $500 | 12 months |
| Medium | Up to $2,000 | 12 months |
| High | Up to $15,000* | 12 months |
*Higher funding amounts for assistive technology may be approved when supporting evidence is provided.
For complex home modifications, the funding period may be extended for another 12 months, providing up to 24 months in total, where the required work has already started.
People with certain progressive conditions may be eligible to access assistive technology funding for 24 months, with possible extensions of up to 48 months where required.
Additional funding or longer access periods may also be available for specific needs, such as an assistance
How Do I Get Funding for Assistive Technology and Home Modifications?
To access funding through the AT-HM Scheme, you’ll first need to complete an aged care assessment.
If you’re approved, your eligibility will be included in your Notice of Decision letter and support plan.
Depending on your needs, you may receive funding for assistive technology, home modifications, or both. This funding is provided separately from your other Support at Home services.
Once your AT-HM funding becomes available, you can start accessing approved products, equipment or home modifications without waiting for your other Support at Home funding.
How Do I Know What Assistive Technology or Home Modifications I Need?
If you’re approved for the AT-HM Scheme, your aged care assessment will help determine whether you need assistive technology, home modifications, or a combination of both.
Your Support at Home provider will discuss your needs with you and recommend suitable equipment, products or changes to your home.
If more specialised support is required, your provider can arrange an assessment with a qualified health professional to recommend the most appropriate equipment or home modifications.
Assistive Technology
Your provider will help you access the right products and equipment based on your individual needs.
They can:
- Source and supply equipment that doesn’t require a prescription
- Arrange an assessment with a qualified health professional, such as an occupational therapist, when a prescription is required
- Work with your health professional to organise and supply any prescribed equipment
Home Modifications
Your provider will help coordinate the home modifications you need to improve safety and accessibility at home.
They can:
- Arrange an assessment with an occupational therapist
- Discuss the recommended modifications and get your approval
- Coordinate the installation with a suitably qualified professional
Can I Buy, Rent or Loan Equipment?
You can buy, rent or loan assistive technology, depending on your needs and how long you require the equipment.
If you only need equipment for a short period, renting or borrowing may be a suitable option. Your provider can help organise the right arrangement for you.
