Managing your own budget doesn't mean managing it alone.

Self-managing your Home Care Package gives you more flexibility and control over how your funding is used, but that doesn’t mean you’re expected to navigate everything by yourself.

Every self-managed client at Gratitude Home Care is paired with a dedicated Care Partner, someone who gets to know you, understands your goals, and helps ensure your support continues to work for you as your needs evolve.

You’ll always have a familiar person to speak with, someone who understands your circumstances and can provide guidance whenever you need it.

Your Dedicated Guide Throughout Your Journey

Unlike large providers where you’re passed between departments or call centres, your Care Partner becomes your consistent point of contact.

They’re there to help you understand your package, answer your questions, and ensure you’re making the most of the funding available to you.

Whether you’re reviewing your budget, adding services, changing support workers or planning for future care needs, your Care Partner is only a phone call away.
