What community access and transport services are available for older Australians?
Older Australians can access transport services to help them get around and stay connected with their community. These services assist with attending appointments, social activities, and other essential outings, promoting independence and social inclusion.
Are these services covered by the Support at Home program?
Yes, transport services are included in the Support at Home program, which replaces the Home Care Packages Program from 1 November 2025. The program offers a range of services to support older Australians in living independently at home and participating in community life.
How can I access these services?
To access transport services, individuals need to be assessed by My Aged Care to determine eligibility for the Home Care Package. Once approved, a care plan will be developed, and services will be coordinated through an approved provider.
Do I need a referral to receive these services?
A referral is typically not required to access transport services through the Home Care Package. However, a comprehensive assessment by My Aged Care is necessary to determine eligibility and appropriate services. Reach out to Gratitude for an accurate, personalised quote.
Are there costs associated with these services?
