How to Switch to Gratitude In-Home Care
Changing providers may seem overwhelming, but it’s a straightforward process. Here’s how to make the move:
Step 1: Notify Your Current Provider
Contact your current provider and let them know you plan to switch. Most providers require written notice (usually 2–4 weeks). Ask for your cessation date and request a copy of your Home Care Agreement and unspent funds balance.
Step 2: Call My Aged Care (1800 200 422)
Call My Aged Care to advise them of your intention to change providers and ask for your unique referral code.
Step 3: Contact Gratitude In-Home Care
Call us on 1300 912 155 and we’ll walk you through the next steps. Share your new referral code with us and agree on a start date. We’ll discuss your needs, explain our services and fees, and start the intake process.
Step 4: Sign Your New Agreement
We’ll prepare your Home Care Agreement and coordinate your transfer with My Aged Care. Once your cessation date is confirmed, we’ll ensure there’s no gap in your support.
Step 5: Begin Care with Gratitude
We’ll work closely with you to ensure a smooth transition. Your services will start promptly, and your care plan will be tailored to meet your current goals and preferences.
