Apply for an Aged Care assessment | MyAgedCare | Gratitude In-home Care

How to Apply for an Aged Care Assessment

To access government-subsidized aged care services for the first time, you’ll need an assessment. You can apply quickly online in just 15–20 minutes or by calling 1800 200 422 to speak with My Aged Care staff.

Online Application Overview

  • Eligibility: Covers all care types, including in-home care (HCP), respite, and residential aged care (RAC).
  • Flexible Access: Apply for yourself or on behalf of a loved one, and set up a support person if needed.
  • Requirements: Have your Medicare card ready.

Note: If you’re already registered with My Aged Care, call instead of applying online.

Application Steps

  1. Confirm Eligibility (5 minutes)
    Answer questions about your situation to check if you qualify for services.
  2. Provide Details (10 minutes)
    Share personal information, including Medicare details. If applying for someone else, include both sets of information.
  3. Arrange the Assessment (5 minutes)
    Specify who the assessment team should contact to confirm needs and schedule an assessment.

After Submission

  • What Happens Next? An assessment team will contact you within 2–6 weeks to finalize your needs and schedule an assessment.
  • Track Your Progress: Log in to your My Aged Care account or call 1800 200 422.
  • Welcome Pack: You’ll receive helpful information and guidance by mail after applying.

The Support at Home program highest level of funding to exceed the current level 4 HCP subsidy by 27%

The Support at Home program will replace the current Home Care Packages (HCP) and Short-Term Restorative Care Programs as of July 1, 2025. The Commonwealth Home Support Programme (CHSP) will join in by July 1, 2027.

This major reform aims to empower older Australians to live independently at home for longer, with more personalised support options and increased funding levels.

✨ More Funding: With 8 levels of funding reaching up to $78,000 annually, there’s up to 27% more than current Level 4 Home Care Packages ($61,440.45/year).

With the highest ongoing classification now exceeding Level 4, if you’re already on a level 4 HCP and require additional support, you could get more funding under the support at home program.

✨ Impact: By 2035, around 1.4 million Australians will benefit from the new program!

✨ Easier Access: Combining CHSP and Home Care Package funding will make it easier for seniors on CHSP to find the right providers and support.

Open Disclosure | Aged Care | Gratitude in-home care

The Aged Care Quality and Safety commission has released a framework and guidance document on the topic of open disclosure.

To summarise, open disclosure is a key requirement in aged care, ensuring transparency when things go wrong in care delivery as per compliance for registered providers.

The Aged Care Quality and Safety Commission audits how well providers apply open disclosure practices. Evidence of open disclosure demonstrates that a provider has effective systems for identifying risks, learning from incidents, and improving care quality. This highlights the provider’s commitment to building consumer partnerships and prioritising safety in care delivery.

 

Principles of Open Disclosure:

✨ Dignity and respect

✨ Transparency in communication

✨ Privacy and confidentiality

✨ Continuous improvement

 

When harm occurs, providers must practice these Elements for open disclosure:

✨ Acknowledge the event

✨ Provide immediate support

✨ Apologise or express regret

✨ Investigate and explain what went wrong

✨ Learn from the event to prevent future occurrences

 

Providers should practise open disclosure in various situations, such as:

✨ Medication errors

✨ Infection control failures

✨ Inappropriate use of restraints

✨ Poor management of personal care, nutrition, or incontinence

✨ Inadequate end-of-life care

 

By embedding open disclosure into their care practices, aged care providers demonstrate a commitment to transparency, consumer safety, and continuous improvement. These efforts not only build trust with consumers and their families but also lead to higher standards of care and accountability across the sector.

Younger People in Aged Care: 2025 Goal | Aged Care | Gratitude in-home care

In 2019, the government committed A$50 million to ensure no individuals under 65 would enter residential aged care by 2022, and none would remain there by 2025. This aimed to address the unsuitability of aged care facilities for younger people, providing them with appropriate housing and support instead.
However, with only six months until the 2025 deadline, achieving this goal appears unlikely. Recent Australian Institute of Health and Welfare (AIHW) data shows that as of the end of 2023, 1,470 people under 65 (excluding First Nations people aged 50–64) still live in aged care.

Several challenges have hindered progress, including the complex needs of younger people in aged care, a shortage of accessible housing, insufficient support services, and difficulties in policy implementation.

To meet the 2025 target, there must be renewed efforts and enhanced strategies. This includes increased funding, stronger partnerships across sectors, community engagement to develop tailored solutions, and a review of current policies to remove implementation barriers.

New Government funded service: Free aged care translation service including sign language

The Australian Government is now providing a free translation service to help aged care providers communicate with older people from culturally and linguistically diverse (CALD) backgrounds.

Aged care providers have free access to TIS National, which includes 24/7 immediate phone interpreting, pre-booked phone interpreting, on-site (in-person) interpreting, Automated Telephone Interpreting Service (ATIS), and video remote interpreting. These services ensure effective communication and improved care for older Australians from diverse backgrounds. Providers can get print and digital materials translated into various migrant languages and use ‘Easy Read’ or ‘Easy English’ versions for better clarity.
